The online reporting form may be used when a traffic collision occurred but the identity of the driver is not known. (Examples: Your car was parked unattended and an unknown driver/vehicle hit your car and didn’t leave a note or a vehicle hit your car and then fled without stopping to exchange information).
Although filing a police report is optional, California law requires that the registered owner of a vehicle involved in any collision in which more than $750 worth of damage occurred must also report the collision to the DMV on a “Statement of Financial Responsibility” form. This form, also known as an “SR-1” form, can be obtained from your insurance company or the DMV.
